The normal opening hours of this practice are between 08:00 am and 6:30 pm Monday to Friday. We offer extended hours, (early and late appointments) on Monday and alternate Thursdays & Fridays, strictly by appointment only.
West Kent CCG provides urgent GP services for our patients outside the normal working week; they can be contacted by telephoning our practice telephone number as above.
HOW TO REGISTER WITH THE PRACTICE
It is necessary to complete an application form. You will be registered as a patient of the practice; however we will allocate you to a list of one of the GPs. This is because we would encourage all patients to consult with their ‘usual’ doctor whenever possible in order to enhance continuity of care.

HOW TO BOOK AN APPOINTMENT FOR A CONSULTATION WITH A DOCTOR OR NURSE
Telephone Woodlands Health Centre, Paddock Wood 01892 833331 between 8:00 am and 6:30 pm. Telephone consultation with the doctor can be requested.
Internet appointment booking and prescription service is available following application and ID check . You can also opt to receive an appointment reminder via text message.
HOW TO REQUEST A HOME VISIT
If you are too ill to attend the surgery and feel you require a home visit, please telephone your request as early as possible, preferably before 10:00 am.
Please hand in your prescription request at reception or use the external post box; and allow three working days notice.
We regret that we are unable to accept telephone requests as this may lead to increased risk of processing error.
The practice is able to offer the facility of repeat prescriptions online. Please refer to the website or pick up a leaflet giving details of how you can register for this service.
Repeat prescriptions can only be obtained during our normal working week as the ‘out of hours’ services are unable to deal with these.
We ask that you telephone between 12 and 3 for results. Alternatively you can register online to view results, book and cancel appointments and request repeat prescription. Please contact the surgery for full details.
For reasons of confidentiality we are only able to give results to the patient, unless under 16 years or in exceptional circumstances. If you wish the result to be given to someone else please inform the doctor or nurse beforehand.
We offer a variety of services in addition to doctors’ and nurses’ appointments. We offer long term condition healthcare including monitoring of asthma, blood pressure, COPD, diabetes, heart disease and hypertension. Our nurses are able to do leg ulcer dressings, wound care, ear syringing, cervical smears etc. Other services include family planning; travel health; minor operations; child immunisations, as well as our seasonal flu, pneumonia and shingles vaccination programmes.
WHAT IF I AM DISABLED?
Access to the Health Centre is by ramp and the doors are wide enough for wheelchairs. We regret that owing to space and safety considerations we do not normally allow mobility scooters onto the premises; if you feel that not being able to do so will make it difficult for you to access our services, you may contact your GP or a member of staff so that your individual circumstances can be considered. We also have a Loop System for those with hearing difficulties.
At East Peckham a ramp is available. Please let our receptionists know in advance so that it is ready for use.
HEALTH CARE IS TEAM CARE
Health care is offered not only by the doctors but also nurses, health visitors, , dentists, midwives, secretaries, receptionists, registration personnel, cleaning staff etc; in all approximately 46 members of staff.
These services are accessed by nurse or doctor referral only.

PATIENT PARTICIPATION GROUP
If you feel that you would be able to contribute to our patient participation group please feel free to contact the surgery for more information. The PPG meets 6 times a year and provides the opportunity for practice staff and patients to discuss topics of mutual interest in our practice with an aim of improving quality. We want our patients to be part of our team.
You may be offered an appointment to see a GP registrar, a fully qualified doctor who is receiving special training for General Practice. Sometimes you will be asked for your permission to video record your consultation for training purposes. You may also withdraw your consent after you have seen the doctor.
Medical and other clinical students also gain experience in our practice in a more supervised way.
PATIENT RIGHTS AND RESPONSIBILITIES
Patients have the right to:
· Be treated with courtesy and respect;
· Be offered a consultation with a doctor on the same working day for urgent problems
Patients have the responsibility to:
· Inform the practice of any changes in contact details;
· Cancel any appointment they are unable to keep;
· Treat health centre staff with courtesy and respect;
· Allow at least 3 working days when requesting repeat prescriptions
ZERO TOLERANCE POLICY
This practice will not tolerate any form of abusive behaviour towards the staff; any such action will be treated seriously and your doctor will be informed immediately, which may result in change or withdrawal of service
This practice endeavors to protect all information about each patient and only shares with those who have a need to know such as other health care professionals who are involved in their care.
We take confidentiality seriously. If you feel that your confidentiality has been breached please contact the Staff Manager, Sarah Baker. If you have an issue that you wish to discuss with the receptionist in confidence please ask to speak to the staff member in our interview room.

We aim to run a smooth and effective service. If you have any suggestions or complaints, please contact the Practice Manager.
ACCESS TO HEALTH INFORMATION
We need to hold personal information about you on our computer system and in paper records to help us to look after your health needs, and your doctor is responsible for their accuracy and safe-keeping. Please help to keep your record up to date by informing us of any changes to your circumstances.
Doctors and staff in the practice have access to your medical records to enable them to do their jobs. From time to time information may be shared with others involved in your care if it is necessary. Anyone with access to your record is properly trained in confidentiality issues and is governed by both a legal and contractual duty to keep your details private.
All information about you is held securely and appropriate safeguards are in place to prevent accidental loss.
In some circumstances we may be required by law to release your details to statutory or other official bodies, for example if a court order is presented, or in the case of public health issues. In other circumstances you may be required to give written consent before information is released – such as for medical reports for insurance, solicitors etc.
To ensure your privacy, we will not disclose information over the telephone or fax unless we are sure that we are talking to you. Information will not be disclosed to family, friends, or spouses unless we have prior written consent, and we do not leave messages with others.
You have a right to see your records if you wish. Please ask at reception if you would like further details and our patient information leaflet. An appointment will be required. In some circumstances a fee may be payable.
FEES FOR NON NHS WORK
Patients sometimes require services not covered by the NHS. Examples are: reports and examinations for school, insurances, employment, private medical care, sport, travel etc. Charges are made for this non-NHS work. A price list is displayed in the waiting area. We accept most card payments.

MINOR INJURIES/ A & E
We are your local minor injuries service.
We are able to deal with sprains, cuts, burns, head injuries. We can stitch, glue, or dress wounds as necessary and assess possible fractures.
